BEYOND THE WHISTLE SPORTS MEDIA DOWNLOAD HERE
WAIVER AND CONSENT FORM
This agreement (“Agreement”) is entered into on this [DATE] by and between the undersigned participant, parent/guardian, and coach, and Beyond the Whistle Sports Media, a sports media company (“Company”).
By signing below, the undersigned acknowledges and agrees to the following terms and conditions:
1. Services Provided
Beyond the Whistle Sports Media provides three main services to the participant:
Highlight Videos: Compilation of notable plays and key moments.
Player Mic'd Up Videos: Videos capturing the player’s in-game audio, including commentary and interactions.
Full Game Footage: Complete recording of the entire game/event.
2. Consent for Mic'd Up Section
a. Player Consent: The player consents to being mic'd up during the game for the purpose of recording in-game audio and commentary. The player understands that the mic’d up portion may capture audio from other players, coaches, and officials.
b. Parent/Guardian Consent: The parent or legal guardian of the player acknowledges and gives consent for the player to participate in the mic'd up recording, and understands recordings may be used for media production purposes.
c. Coach Consent: The undersigned coach confirms that they have reviewed this agreement and consent to the player being mic'd up during the game for the purpose of capturing in-game audio.
3. No Performance Guarantee
The Company does not guarantee any specific outcomes, including but not limited to:
The player’s performance during the game, the amount or quality of commentary or audio that the player will contribute during the mic’d up section.
4. Use of Media
By signing this agreement, the participant, parent/guardian, and coach grant Beyond the Whistle Sports Media the right to use the videos, including highlight reels, player mic'd up videos, and full game footage, in any media format (including but not limited to social media, websites, promotional content, and advertisements) without further consent or compensation.
